Nomination Form for Twenty-fourth Annual
Outstanding Teacher Awards
2011–2012

The Technology Alliance of Central New York has established a teacher awards program for the advancement of mathematics, science, and technology education for the elementary and secondary students of the five counties of Central New York: Cayuga, Cortland, Madison, Onondaga, and Oswego.

Up to five teachers will receive cash stipends of $1000 each plus a distinctive plaque. In addition, the recipient's school districts will receive a companion plaque at local ceremonies during the spring of 2012. The recipient teachers will be honored at a Technology Alliance Awards Dinner & Program on May 4, 2012.

The criteria for the awards require documentation that the teachers:

• Inspire students to pursue study in fields related to mathematics, science, or technology
• Encourage students to demonstrate outstanding academic achievement
• Involve students in co-curricular activities such as mathematics, science, or technology exhibits or competitions
• Serve as role models for students in the responsible use of technology
• Demonstrate active participation in professional development
• Share their expertise and enthusiasm with colleagues

Nominees must teach students who live in one of the Central New York counties noted above, and must have taught in the same district for the three school years prior to nomination. Previous recipients are ineligible for five years following the year in which they received the award. Following nomination, teachers and their nominators will be asked to supply additional information and references. The complete file will be due by Monday, November 28, 2011.

Nominees will be reviewed by the Teacher Awards Committee, which is made up of persons from business, industry, and education. Final selection will be made during January 2012, and notifications will be made as soon as possible thereafter.

An information form will be sent to the teacher for completion after this nomination is received by the committee.
