Nomination Form for Teacher Awards Program

Nomination forms and invitations to nominate teachers are mailed in early September to administrators in all school buildings in the five counties of Central New York and to various other individuals in TACNY and the technology community. Notices are also sent to the daily and weekly newspapers and electronic media of the region. The nomination form may be downloaded in PDF.

Nominations must be received by Monday, October 10, 2005.

After a teacher has been nominated, an information form will be sent to the nominee. The nominated teacher must complete that form with information pertaining to the criteria for the awards and also request three letters of support from supervisors and colleagues. Information forms may be downloaded in PDF.
